Health clearances are required for all students attending Honolulu Community College. Plan to submit these documents early to avoid any delays.
Department of Health Requirements
The State of Hawai’i Department of Health (DOH), the Hawai’i Administrative Rules, Chapter 11-157 requires all students to meet health requirements before they attend any post-secondary institution in the State of Hawai’i.
All college students are to be free of:
- Tuberculosis (TB)
- Tetanus-diphtheria-pertussis (Tdap)
- Varicella (chickenpox) and
- Measles, Mumps, and Rubella (MMR)
Your Department of Health (DOH) immunization records should be submitted to Med+Proctor, the University of Hawaii’s partner for health immunization record processing and review. You will need to set up a Med+Proctor account with your UH email account.

Measles/Mumps/Rubella (MMR) Clearance
Required
- Proof of two (2) Measles, Mumps, Rubella (MMR) shots is required.
- One recent MMR immunization is acceptable for registration, a second immunization record is required 45 days after the first immunization shot.
- Titer (blood work) is not acceptable.
- International (F-1) Students – MMR immunization from a student’s home country is acceptable as long as a M.D. in the U.S. reviews and signs off on the foreign immunization record for clearance.
Additional Info
If a student is born before 1957, an MMR immunization record is not required.
Tuberculosis (TB) Test Clearance
Acceptable Immunization Forms
- UH Health Clearance Form
- TB clearance card
- TB Report (record must have TB test date, TB read date, millimeter test reading
- a negative x-ray report, or a completed Risk Assessment Form
- International (F-1) Students: TB Clearance must be from a M.D. in the U.S. before registration is allowed.
Additional Info
New Students: Must have a negative TB skin test or x-ray completed at the age of 16 or older, or a report within 1 year of the start of the semester. Students can also submit a Risk Assessment Form directly from their physician’s office or the Department of Health.
UH Manoa Students: Send a signed written memo requesting copy of TB & MMR health clearance documents to be sent to Honolulu CC via campus mail or for pick-up. Include your Banner ID#, name, and phone number to University Health Services at Manoa (Fax # 808-956-0853, Office # 808-956-8965). Allow a minimum of 1 week to process your request.
Tetanus-Diphtheria-Pertussis (TDAP) Clearance
Required
- Proof of one (1) shot of Tdap (administered at the age of 10 or older) is required. Note: Dtap, Dt, and Td are not the same thing as Tdap.
- International (F-1) Students: Tdap immunization from a student’s home country is acceptable as long as a M.D. in the U.S. reviews and signs off on the foreign immunization record for clearance.
Varicella (Chickenpox) Clearance
Required
- Proof of two (2) shots of varicella is required.
- One recent varicella immunization is acceptable for registration, a second immunization record is required 45 days after the first immunization shot.
- A documented history of varicella (chickenpox), signed by a U.S. licensed physician, APRN, or PA may be substituted for the varicella vaccine requirement.
- Titer (blood work) is not acceptable.
- International (F-1) Students: Varicella immunization from a student’s home country is acceptable as long as a M.D. in the U.S. reviews and signs off on the foreign immunization record for clearance.
Additional Info
If a student is born in the U.S.A before 1980, a varicella immunization record is not required.
